Adrienne has worked in the Social Services field for 21 years in various clinical and healthcare settings serving older adults, rehab patients, persons with acquired brain injury and mental health needs, hospice, and families with young children. She got her start working with older adults as a Social Services Director in various skilled nursing facilities in the South Bay and found her true calling in working with older adults. In her many years of experience, Adrienne has provided skilled and detailed assistance to clients in developing and implementing care plans; providing supportive bereavement counseling; hospital and rehab discharge planning; and facilitating training and multidisciplinary team meetings. Adrienne feels that paramount to a successful relationship between care manager and client is forming a real connection and ensuring her clients’ needs and concerns are heard. She works hard to authentically validate her clients’ feelings and experiences. Adrienne feels most passionate about working with older adults facing dementia and complex medical conditions, helping to safely support their independence while encouraging ongoing growth and engagement.
