Celebrating 25 Years: How Integrated Care Transformed Vernon’s Journey
We are honored to share Vernon’s story to kick off our 25th year of serving older adults and their families.
As one of our long-term clients, Vernon has experienced the unique benefit of receiving all three of our services—care management, home care, and The Hummingbird Project. Vernon’s journey with Sage began more than a decade ago. Today, Care Manager Adrienne has been by his side for 13 years, coordinating medical appointments, managing mental health resources, and serving as his advocate. “We are his family,” Adrienne says. She ensures that Vernon’s big-picture needs are met, offering stability and guidance that keep everything else running smoothly. Her consistent presence reassures Vernon that he is never alone in navigating life’s challenges.

Christine (pictured above), Vernon’s personal care attendant (PCA), is equally vital. She provides practical assistance and companionship through outings, shared meals, and day-to-day support. With Christine, Vernon has someone he trusts by his side to share in the small joys of daily living. Whether it’s a walk in the community, a trip to a favorite restaurant, or simply enjoying conversation, Christine’s presence has given Vernon the gift of stability and human connection.
One example stands out: Christine accompanied Vernon to a restaurant on the coast for his birthday. What might seem like an ordinary dinner became extraordinary to him. He later described it as “the best birthday of his life.” With Christine’s support, birthdays shifted from painful reminders to joyful celebrations.
The Hummingbird Project: Enrichment and Purpose
The Hummingbird Project has brought creativity and inspiration into Vernon’s weekly routine. Jen, his engagement specialist, has worked with him since 2017. She designs activities that stimulate his mind and nurture his spirit. From meditation and affirmations to outings and creative exploration, these sessions provide Vernon with moments of joy and self-expression. “He tells us all the time that we give him meaning and purpose,” Jen shares. “That’s not something you can measure with numbers—it’s about relationships.”
Every Saturday, Jen sends Vernon affirmations and meditations. This small ritual has become a cornerstone of his week, reminding him of his strength and offering comfort. These practices, paired with regular in-person engagement, have helped Vernon develop tools for resilience and self-reflection.
Collaboration in Action
What makes Vernon’s experience remarkable is the quality of each service and the way they come together. Adrienne communicates regularly with Christine and Jen, ensuring that updates are quickly shared and everyone remains attuned to Vernon’s needs. This seamless collaboration allows the team to respond promptly when challenges arise and to celebrate victories together.
When Vernon faced moments of difficulty, Adrienne’s oversight, Christine’s companionship, and Jen’s encouragement combined to help him move forward. Together, they’ve created a rhythm of care that is steady, adaptable, and deeply personal. Vernon feels supported from every angle: medically, emotionally, socially, and spiritually.
A Fuller Quality of Life
The impact of this trifecta of services is evident in Vernon’s life today. He once struggled with loneliness and uncertainty; now, he thrives in a web of meaningful relationships and routines. He has rebuilt his relationship with his brother, sharing meals and attending church together—connections he treasures deeply. He expresses gratitude by sending thoughtful gifts and notes to his care team, gestures that reflect his sense of belonging.
Most importantly, Vernon has discovered joy in daily living. Integrating care management, home care, and The Hummingbird Project has given him stability, purpose, and companionship. His story shows that whole-person care is not just about meeting needs—it’s about helping people flourish.
